Wheelchairs are the transportation needs of people with physical disabilities and mobility difficulties, and they are also an indispensable tool for family members to move and take care of patients. When in a wheelchair, the main load-bearing parts are the ischial tuberosity of the hip, femur, popliteal fossa and scapula and the surrounding soft tissue. To choose a suitable wheelchair, we should not only consider the material, style and price, but also make individual adaptation according to the patient's figure and needs.
When choosing a wheelchair, consider the following aspects
1. Wheelchair width
After sitting on the wheelchair, the user should have a gap of 2.5~4cm between the two thighs and the armrest, about the width of the second-hand finger. If the seat is too wide, the arms stretch too much when pushing the wheel ring, it is easy to fatigue, and the body cannot be kept stable. It is also inconvenient for the wheelchair to pass through the narrow space; if the seat is too narrow, it will compress or wear the skin of the buttocks and the outer thighs. The lower wheelchair is also cramped.
2. Length of wheelchair
After sitting on the wheelchair, the front edge of the seat surface should be 6.5cm from the back of the user's knee, about 4 fingers wide. If the seat is too long, it will resist the back of the knee and compress the blood vessels and nerve tissue, and wear the skin; if the seat is too short, it will increase the pressure on the buttocks, causing discomfort, pain, soft tissue damage and pressure sores. For patients with short thighs or hip and knee flexion contractures, it is recommended to use a short seat.
3. Height of backrest
Under normal circumstances, the upper edge of the backrest should be about 10cm under the armpit, about palm width. The lower the backrest, the greater the range of motion of the upper part of the body and arms, and the more convenient the functional activities, but the smaller the supporting surface, which affects the stability of the trunk; on the contrary, the higher the backrest, the larger the supporting surface, and the better the stability, but it will affect the functional activities. For patients with poor neck control, it is recommended to use a wheelchair with a high backrest and headrest.
4. Height of seat and footrest
The two should be coordinated with each other, such as the seat is high, the pedal is relatively low, and the pedal is high. Under normal circumstances, the user sits on the wheelchair with his lower limbs placed on the pedals. At this time, the front 1/3 of the lower thigh is about 4cm higher than the front edge of the seat surface and is about 2 fingers wide. If the seat is too high or the foot pedal is too low, it will cause the lower limbs to lose their support, the lower limbs are suspended, and the body cannot maintain balance; on the contrary, the seat is too low or the foot pedal is too high, which will cause the buttocks to bear all the gravity, causing discomfort and damage to the soft tissue of the buttocks for a long time; At the same time, the user will be very difficult to operate the wheelchair.
If the seat and foot pedal are too high or too low at the same time, it will increase the difficulty of the elderly in operating the wheelchair. The former is that the wheelchair cannot lean into the desktop, affecting functional activities, while the latter is easy to collide with the ground and hinder action.
5. Armrest height
When sitting down, the upper arm is vertical, and when the elbow joint flexes 90 degrees, 2 .5cm above the lowest point of the elbow joint is the appropriate height of the armrest. Proper armrest height helps to maintain correct body posture and balance, and allows the upper limbs to be placed in a comfortable position. The armrest is too high, the upper arm is forced to lift, susceptible to fatigue. If the armrest is too low, you need to lean forward to get support, which is not only easy to fatigue, but also may affect breathing. If you have upper limb paralysis and difficulty in maintaining arm posture, it is recommended to use a wheelchair table to support your upper limbs.
